BlueJay Wellness Center is a Child and Adolescent Health Center that promotes the health of children, adolescents, and their families by providing important primary, preventative, and early intervention health services.

Our team provides expert medical and mental health care by appointment for patients age 3 to 21 within Shepherd School District.

​

Services include primary care, preventative care, comprehensive health assessments, vision and hearing screenings, medication, immunizations, treatment of acute illness, co-management of chronic illness, health education, and mental health care.

Services can be billed to health insurances if applicable. No eligible person will be denied services based on insurance status or ability to pay. Staff can assist with Medicaid enrollment.
